NOTICE FOR PARENTS OF MHS ATHLETES
All student athletes are required to have a sports physical and a completed Athletic Handbook each calendar year, in order to participate in any school sport.
The protocol to complete the physical requirement is to make a wellness visit appointment with pediatrics or family medicine at Mammoth Hospital (or the hospital of the athlete's choice) and bring with you, the school physical clearance paperwork located on the MUSD website.
With The Affordable Healthcare Act, all insurance companies are REQUIRED to provide a once yearly wellness visit to every patient. The yearly physical is FREE OF CHARGE.
If an athlete is UNINSURED, he or she may become insured as a child (under 18). That process is true regardless of a student's citizenship status. This is important, because a physical for an uninsured athlete will be VERY COSTLY. I am happy to connect any athlete's family with information on how to become insured.
Thank you. Please do not hesitate to contact Heidi Thompson with any questions regarding requirements for high school athletic participation.
