  A parent or guardian MUST complete the Pre-Participation Physical Evaluation Form before the exam. Download it here English Spanish (PDF), print it, fill it out, sign it, send with your student to the exam.

  
  
  NOTE: Mammoth Youth Football and Cheer will also be participating in this clinic. MYFL participants must be accompanied by a parent/guardian and must bring the Pre-Participation Physical Evaluation Form, completed and signed by parent/guardian, with them. High School students do not have to have a parent/guardian with them at the exam.
• Mammoth High School Makes Newsweek Top School List
  Every year Newsweek Magazine publishes a list of the "top" high schools in the United States. This year they ranked the top 1622 schools, 6% of America's public high schools. The rankings are based on how hard school staffs work to challenge students with advanced-placement college-level courses and tests. This year Mammoth High School ranked 1065 putting it in the top 4% of all US public high schools by this ranking.
